Before ANY athlete can participate they must have ALL paperwork/online forms completed:  

1). OSSAA Sports Physical (click on link for OSSAA Physical form)  

2). Rank One Sport Paperwork (www.rankonesport.com)  

  (Physicals are good for one year from May 1st. Complete Rank One Paperwork after June 1st to be effective for the following school year.)
